To help you save time starting at your very first appointment, you now have the option to fill out our patient forms before you’re sitting in our office. Just click on the link below to fill out our forms and complete them online in the comfort of your own home. When the day of your visit arrives, we will have all the information we need, and we’ll get you in to see Dr. Miller right away. If you have any issues accessing our forms, be sure to let us know!
Dental care should not be a hassle, and it should not be expensive for patients to enjoy receiving the best dental care possible. At our office, we accept most major dental PPO insurance plans. Additionally, we are an in-network provider for Unum Dental PPO, BCBS Grid+, as well as Always Care.
As a courtesy, we will file your insurance claim on your behalf to help make your experience even more convenient and stress-free. However, we do ask that you be prepared to pay your portion of the bill at the time services are rendered.

If you currently do NOT have dental insurance or require a treatment that is not fully covered by your plan, flexible, low-to-no interest financing is available through CareCredit.
